Licensed Practical Nurse (LPN)

Our LPN is the clinical backbone of the home. You’ll oversee medication administration, vitals monitoring, wound care, and the daily coordination with physicians, specialists, and pharmacies. You’ll also train and support our DSPs, write and review care plans alongside our Director of Nursing, and be the steady clinical voice families rely on. Small caseload (six residents), long relationships, real continuity of care — not the churn of a large facility.

What you'd be doing

• Administer medications and maintain MAR documentation
• Monitor vital signs and residents’ daily health status
• Coordinate with physicians, specialists, pharmacies, and hospital discharge teams
• Contribute to quarterly care plan reviews
• Train and support Direct Support Professionals on clinical protocols
• Respond to health changes with clinical judgment and clear family communication

What we're looking for

• Active Arizona LPN license in good standing
• Minimum 2 years LPN experience; developmental disability or long-term care preferred
• Current BLS / CPR certification
• Clean fingerprint clearance card
• Comfort with electronic health records and clear clinical documentation
• Valid AZ driver’s license
